Form Reports

Form Summary

The Form Summary is a set of Reports visible within the Form management page.

Note: The Form Summary is not a printable Report.

  • On the Form Summary page, you'll see up to three pie charts:
    1. Number of Responses submitted.
    2. Number of Responses with payment.
    3. Total amount paid.
  • Click the charts to get quick access to the Responses (i.e. click the unpaid section to access Responses of people who have not yet paid).

  • The answers to each question on the Form are then graphically displayed under the Summary tab.

Text Fields

  • Text fields include single line and paragraph style questions.
  • These fields will display a separate line for each Response that answered the question.
  • If no Profile fields are included on the Form, 'anonymous Response' will be displayed instead of the name.

Date Fields

  • The summary will show a timeline on the left with each date plotted on the line as well as a list of the actual dates on the right.
  • In addition to seeing the number of people who answered the question, you will see the percentage of Responses that answered the question in a specific way.

Number Fields

  • In the summary, like numbers will be grouped together with a bar graph displaying the chosen numbers on the left and number of Responses with percentages on the right.

Radio Buttons

  • In the summary, the answer will be displayed as a pie chart on the left, with specifics on the right.
  • Hover over the pie chart to see information about each part.

Checkboxes

  • The summary will display the answers as a bar graph on the left with specifics on the right.

File Upload

  • The summary shows each Response with a file connected, along with the ability to download the file from this page.
  • Click the file name to download the file.

Prioritize

  • On the left side of the summary section you will see each choice represented by a different shade of gray.
  • Hover over each color to see the total number of Responses that chose that option
  • On the left you will see how many Responses prioritized an item and the percent of total Responses that prioritized the item.

Scale

  • On the left you will see each choice represented by a different shade of gray.
  • Hover over the color to see how many Responses selected that option on the scale.
  • The right side contains the numbers of Responses and the percent of total Responses that picked any of the scaling options.

Product/Ticket

  • The set value and what the responder chooses is what will show under the Form summary.

Donation

  • In the summary section the left side shows a bar graph representing the amounts given to each category.
  • On the right you'll see the total amount given to each as well as how many Responses gave to each category.

Discount Codes

  • On the left you'll see a pie chart representing the number of Responses that used the codes.
  • A list of discount codes, the type of discount, amount discounted and number of Responses that used the discount codes will be shown on the right.

Export Responses

This Report exports the all the Responses to an Excel file.

  1. To access this Report, open the Form.
  2. Click the Reports icon.
  3. Select Export Form Responses.

The Report contains

  • Name of the Form and number of Responses.
  • The date the Response was submitted and last edited as well as the id and confirmation code for the Response.
  • Information about the responder such as individual id (if matched) and Profile information (whatever was included on the Form).
  • Each question from the Form along with the responders answers and, if there is cost involved, the payment status.

Check-in Sheet

The Check-In Sheet allows you to see who has or has not paid and give you quick access to the confirmation code to use for Form check-in.

Keep in mind that if you are using one Form to register for multiple events, you may see a person more than once. A separate line will be shown for each event the person registered for.

  1. To access this Report, open the Form.
  2. Click the Reports icon.
  3. Select Check-In Sheet.

The Report contains

  • The name of the Form and total number of Responses submitted.
  • If there are no Responses to the Form or all Responses are anonymous, a message will inform you of this.
  • The name, date and time of the event(s) connected to the Form.
  • The confirmation code assigned to the Response and the number of people registered for each event.
  • The Event column will only show if the Form has Responses with automation to an event, and the Confirmation Code will only show if confirmation codes are turned on for that Form.
  • Contact information included on the Form (first and last name, contact phone, and email address).
  • Number of tickets purchased, total amount on the Form and payment status.
  • The (ticket) Quantity column will only show when the Form has at least one Response with an answer to a product/ticket question automating to an event.
  • The Form Total and Payment Status columns will only show when at least one Form Responses includes payment information.

People Search

In a People Search, you can include Form involvement in the search criteria.

Privilege Restriction: Only Full and Limited Read Admins may use the People Search.

  1. Click on People in the left navigation menu.
  2. Ensure you are using the Old People Search Page (check out this article to learn more).
  3. Click the Magnifying Glass icon to start your search
  4. Chose the criteria *Form Question Answer.

Warning: Only matched Form Responses will be considered in this people search.

Warning: Only Forms with at least one question in addition to the Contact fields will appear as options.
